Let the Customer Service Center at the Recreational Sports Facility assist you with all your passport needs. 

Traveling outside of the U.S?

The Customer Service Center is an authorized U.S. Government Passport Acceptance Facility and accepts the DS-11 application for the US Department of State. We offer products and services for both first-time and renewal passport applications. 

Passport services are available to everyone – you do not need to be affiliated with UC Berkeley to take advantage of these services.

Hours of Operation

The Customer Service Center is currently open Monday-Friday, 9:00 am-6:00 pm (PST.) and Saturday, 9:30 am-5:00 pm, on an appointment-only basis; no walk-ins. Passport services will be unavailable on all UC Berkeley administrative holidays.

The Customer Service Center will be closed on Thursday, November 21, from 11 am to 12 pm (PST) and November 28-December 1.

Passport appointments are limited to 30 calendar days in advance. If the schedule is fully booked, please continue checking as appointments may become available. We do not offer passport photo services at this time.

Passport Application Fees

Please note that two separate payments are required:

  1. US Passport Fees: Payable to the “US Department of State” via cashier’s, traveler’s, or personal check or money order only. Starter checks are not accepted. These fees include the passport application fee and any optional expedited service fee or 1-2 Day Delivery fee. Prepare one check or money order per applicant, including the applicant’s name and date of birth on the memo line. Calculate passport fees online and contact us if you have additional questions.
  2. Passport Acceptance Facility Fee: Pay this separate fee in person at your appointment. Accepted payment methods include check (payable to “UC Regents”), cash, debit, or credit card for the required $35 processing fee. An optional $30 fee is available for overnight mailing from UC Berkeley to the Department of State.

Passports for Minors Under 16

Passport applications for children under 16 have special requirements.

  • The applicant (minor) and both parents/guardians must be present to authorize the issuance of a child’s passport. Please visit the State Department website, item #7, if both parents/legal guardians cannot appear in person.
  • Children need evidence of citizenship.
  • Parents/legal guardians need proof of parental relationship.
  • Note: Children’s passports are only valid for 5 years.

Lost or Stolen Passports

If your passport has been lost or stolen, you should report it immediately to the U.S. Department of State. Report Your Lost/Stolen Passport (form DS-64).

Reporting your prior passport lost or stolen does not replace it. Schedule an appointment with us to replace your passport and submit form DS-64, form DS-11, and other required documents when you apply in person.
