Lost or Stolen Passports
If your passport has been lost or stolen, you should report it immediately to the U.S. Department of State. Report Your Lost/Stolen Passport (DS-64 form).
Reporting your prior passport lost or stolen does not replace it. Schedule an appointment with us to replace your passport and submit a DS-64 form, DS-11 form, and other required documents when you apply in person.
