U.S. Passports

Apply for your passport several months before your trip. First-time applicants, minors, and applicants who need a new passport, but may not renew by mail, may submit their passport application at the Customer Service Center. The U.S. Department of State website further explains what you'll need to bring with you.

• DS-11 Form (completed and unsigned)
• Evidence of U.S. Citizenship (original or certified copy) and a photocopy.
• Valid Government-Issued ID and photocopy of the front and back.
• U.S. Passport Photo
• Passport fees

• Completed DS-11 Form
  Verify your printer settings are fit-to-page and print the completed form on single-sided, 8.5 x 11-inch paper.  If you do not have access to a printer, please arrive prior to your appointment to complete the form in person. Do not sign the application until instructed—a Passport Acceptance Agent must witness your signature.

• Submit Proof of U.S. Citizenship

  Applicants must submit an original or certified copy of your proof of U.S. citizenship. The original document will be sent to the U.S. Department of State with your application. It will be returned to you in a separate mailing 1-8 weeks after you receive your passport.

• Present Valid Government-Issued Photo ID

• A Printed U.S. Passport Photo

• Prepare a Check or Money Order payable to the "US Department of State" for the U.S. Passport Fees (See Step 4)

• Prepare a separate $35 acceptance fee is required for each DS-11 application, payable to "UC Regents". This fee is payable with cash, card, check or tap-to-pay options during your appointment.

• Check for Special Circumstances or Requirements
  Certain applicants may need additional forms or supporting documents. Review your eligibility and gather additional requirements before your appointment.

We process passport applications on an appointment-only basis, no walk-ins. Please schedule an appointment before visiting the Customer Service Center.

DS-11 passport applications have two types of basic fees, the passport fee, and the application acceptance fee. Please note that two separate payments are required to process an application at a Passport Acceptance Facility.

U.S. Passport Fee - Department of State

This payment is sent along with your application to the Department of State for processing. Department of State fees are paid separately from Customer Service Center fees and are payable to the “U.S. Department of State".

• US Department of State Fee: Fees can vary. Calculate passport fees online.
  
• Payment methods: You must pay this fee with a check or money order only.
  • Personal, certified, or cashier's check, payable to the "U.S. Department of State." Starter checks are not accepted. 
  • Money order payable to "U.S. Department of State." 
  • Prepare one check or money order per applicant, including the applicant’s name and date of birth on the memo line.

Application Acceptance Fee – Customer Service Center

Pay this separate fee in person at your appointment. Accepted payment methods include card, cash, tap-to-pay options, or check payable to "UC Regents".

• $35.00 - Required Passport Acceptance Fee (also known as a Passport Execution Fee)
• $30.00 - Priority Mail Express envelope from UC Berkeley to the Department of State (optional)

Passport Acceptance Facilities are able to process passport renewals, using the DS-11 form, for:

• Minors (additional special requirements apply)
• Applicants who were under 16 when their most recent passport was issued
• Applicants whose most recent passport was issued over 15 years ago
• Applicants who have reported their most recent passport as lost or stolen (along with a DS-64 form)
• Applicants whose most recent passport was damaged or mutilated (a note indicating how the passport was damaged is required)

These applicants must prepare the same application package as a first-time passport applicant. If the most recent passport is still valid, applicants will be required to submit it along with the application. Additional proof of U.S. Citizenship (original or certified copy) is required for those whose most recent passport was lost, stolen, or damaged. 

We process passport applications and renewals on an appointment-only basis, no walk-ins. Please schedule an appointment before heading to the Customer Service Center.

You may be eligible to renew your passport by mail, using the DS-82 form, if:

• Your most recent passport was issued within the last 15 years.
• You were 16 years or older when your most recent passport was issued.
• You have your most recent passport in your possession, and it is undamaged (normal "wear and tear" is acceptable).
• Your most recent passport must not have been reported lost or stolen at any time.
• Your previous passport was issued in your current name (or you can provide a document like a marriage certificate or divorce decree to show you have changed your name)

If you complete a DS-82 renewal by mail form, you must mail your application, prior passport and/or card, check, and photo directly to the U.S. Department of State. We recommend using a trackable shipping method or a Priority Mail envelope to the designated P.O. Box listed on page 3 of the application. DS-82 renewal forms cannot be submitted to an Acceptance Facility.

We understand that some applicants prefer to submit their form in person. If you need to renew your passport and plan to do so at our facility, please note that you will need to complete a DS-11 form. We process passport applications and renewals on an appointment-only basis, no walk-ins. Please schedule an appointment before heading to the Customer Service Center.

As a Passport Acceptance Facility, the applications submitted will follow the US Department of State’s processing times. Routine processing is 4-6 weeks and Expedite processing is 2-3 weeks.

Processing times only include the time your application is in one of the U.S. Department of State’s passport agencies or centers. Mailing times are not included in processing times. The total time to get your passport includes both processing and mailing times.

• You are applying for your first passport.
• You were under 16 years old when you received your last passport.
• Your most recent passport was issued more than 15 years ago.
• You do not have your most recent passport in your possession.

Adult Renewals (DS-82 Form)

• Your most recent passport was issued within the last 15 years.
• You were 16 years or older when it was issued.
• You have your passport in your possession, and it is undamaged.
• Your passport has not been reported lost or stolen at any time.
